If you wish to retire and receive a pension check, you can download an Application for Pension including Disability Pension. A second set of forms will be mailed to you once this initial application is received and reviewed.

If you retire and wish to have your health and prescription card coverage withheld from your pension check, please print this Authorization for EWTF Premium Deduction.

A beneficiary form is also available for this Fund.
