College Students
The Electrical Welfare Trust Fund (EWTF) has revised the procedure for certifying that your dependent(s) over age 19 (but less than age 23) are entitled to continued coverage based on their status as full-time students.
An annual Statement of Student Certification is now acceptable in lieu of requesting certification for the fall and spring semesters and a Summer Letter of Intent.
This statement is good for one year. By signing the form you and your college student are agreeing that your student has full-time status for the defined period. Dependent eligibility is contingent upon the member meeting eligibility requirements of the Plan.
NOTE: If your student intends to be a full-time student for the fall semester, she/he is considered full-time for the summer months.
You are also agreeing to the following:
· You are responsible for notifying the Fund office the date your student graduates.
· You will contact the Fund office if your student does not meet the school’s requirements for full-time status.
· You will contact EWTF if your student is not a full-time student for either the spring or fall semester (or a specific quarter).
· You will reimburse the Fund the amount paid on health claims incurred by your student including the prescription card benefits if you fail to notify the Fund office of a change in your student’s status.
Once the Fund office is contacted about a change in status you and/or your student will be notified of the options available for your student to continue coverage with EWTF.
