Benefit Applications
You must complete a Separation Benefit Application if you have experienced a permanent separation from covered employment and if you want to collect this benefit. You must be under age 70 1/2 and the benefit is less than $5,000.00.

If your benefit is more than $5,000.00 and you have experienced a permanent separation from covered employment, contact the Fund Office to find out what you need to do to collect your benefit.

Beneficiary Forms
Anyone who is active in the Individual Account plan receives a welcome package that includes among other things a Beneficiary designation form. If you wish to change your beneficiary, complete this two-sided form and return it to the Fund Office. If more information is required you will be contacted.
