Eligibility for Surviving Spouse/Dependent(s) of Active Participants (this does not include Active Non-Bargaining Unit Employees).

Your dependents can use all of the hours in your hour’s bank to maintain your coverage. Beginning the first of the month when those hours are exhausted, coverage will be extended for twelve months at no cost.

Certain restrictions, limits, pre-authorizations, deductibles and co-pays apply.  Refer to the Summary Plan Description (SPD) for details.  If there is a discrepancy between the SPD and the Plan Document, the Plan Document governs.

Eligibility for Surviving Spouse/Dependent Coverage of Retired Participants and Active Non-Bargaining Unit Employees.

If you and your spouse are covered under this Plan and you die, your spouse is eligible for the Surviving Spouse Coverage through this Plan. Surviving Spouse Coverage begins on the first day of the month after the month that coverage for your spouse would otherwise end. After the three-month extension, your surviving spouse may self-pay for continuation coverage for the remainder of their lives, provided they are not eligible for coverage under any other group health plan other than Medicare.  Coverage is subject to the special rules for a surviving spouse of a deceased participant, as explained in the current SPD booklet.
